Modplug can export to MIDI - it's how I make MIDIs - but I have to admit that it's not very good at it. Only the basic notation is saved, so you lost any volume changes, slide effects, vibrato and so on.

A lot of MIDI cards don't support all of the controllers, so you wouldn't miss out on much.

JonWoG: He said "or maybe PC" which is why I mentioned two things.

It's true what Chris says, not all sound cards support the effects you get from XM/IT/MOD software, so it's best not to use them unless you want your music to sound bad on some systems. Since MIDI's are controlled by hardware instead of software like other formats, you cant prevent this.
